What we collect, and why
The “Request a visit” form
When you use the form, the details you type (your name, phone number, and anything else you choose to add) are emailed to the office so we can call you back. The website itself does not keep a copy. Please don’t include medical details in the form; we’ll talk about those by phone or in person.
Cookies
Visiting the site doesn’t require cookies. The form may set a short-lived technical cookie so it can be sent securely. If you log in to manage the site, WordPress sets login cookies.
Embedded and linked content
Some pages link to other websites (Google Maps, Google reviews, Yelp, Facebook, Instagram and others). Those sites have their own privacy policies and may collect data about you when you visit them.
Your health information
Health information you share with the practice as a patient is protected under our Notice of Privacy Practices, which is posted in the waiting room in Ramsey and available from the front desk.
Your rights
You can ask us what personal information we hold about you, and ask us to correct or delete it, by calling or writing to the office.
